Prior to attending practice all players must have fulfilled the KPBSD Requirements. You must be enrolled in Planet HS to show that you have met the physical, TAD, Concussion and Parental forms. I am also including two forms – one for Demographics and one for information on the communication that should be kept handy at home. These should be returned to me as soon as you can but do not need to be filled in to practice. These can be downloaded and brought to the first practice. They can be found on the Team Page
